The rehabilitation and nursing centers of New Hampshire Catholic Charities provide exceptional care that meets the physical, psychological, and spiritual needs of all those they serve. The centers offer residents and short-term patients a warm and friendly atmosphere where they are cared for according to their individual care needs including; long-term care, dementia/Alzheimer's care, and skilled nursing/short-term rehabilitation.
Skilled Nursing and Rehabilitation Services
Each center provides rehabilitation therapy and nursing care for patients after hospitalization. Such "post acute" care is often needed after today's shorter hospital stays and may be appropriate following:
General or cardiac surgery
Joint replacement and orthopedic surgery
Traumatic and brain injuries
We also provide complex nursing care for:
Specialized skin and wound care
Nutritional support and diabetes management
Alzheimer's & Dementia Care
People with Alzheimer's or similar type memory impairment usually benefit from a structured routine and supportive social environment separate from other long-term care residents. Special secured units are available for residents who are able to participate in and benefit from structured activities.
Short-Term Respite Care
The rehabilitation and nursing centers provide short-term respite care when an at-home caregiver is temporarily unavailable or in need of personal time away.
Individuals with chronic illness or late-stage Alzheimer's disease are cared for with the goal that they attain their highest functional ability. Individualized care programs provide restorative and rehabilitation services as appropriate to achieve that goal.
Hospice or "palliative" care is available if curative or restorative care is no longer appropriate and desired for terminally-ill individuals. Hospice care focuses on providing comfort and pain relief as well as pastoral support and counseling for patients and family. Some centers have a private hospice room that is furnished to accommodate extended visits by family members.
Other Professional Services available include:
Pastoral and religious services
Discharge planning consultation and coordination of home care service
Mental health services are provided by geriatric psychiatrists, psychologists, and psychiatric nurse consultants
Licensed dental, podiatry, optometry, and audiology consultants
Beauty and barber services on site
Assistance with completion of Medicaid applications
SENIOR LIVING COMMUNITIES
In addition to the rehabilitation and nursing centers, New Hampshire Catholic Charities' Senior Living Communities offer service-enhanced, independent living in a comfortable and secure setting. By providing maximum independence in a warm and supportive atmosphere, they are the perfect place for people to maintain their freedom while relaxing and enjoying life. Three buildings are conveniently located in Dover, Laconia, and Manchester.
New Hampshire Catholic Charities offers a variety of services to the elderly across the state. In addition to nursing and rehabilitation centers, we assist the elderly, their caregivers, and loved ones through counseling, support groups, and information and referral services. Catholic Charities can also assist when an elderly spouse is forced to make difficult decisions alone regarding the care of their husband or wife. Through our trained professional staff we provide detailed information regarding options and planning.
